Once the student is enrolled, the family should contact Michelle Cook (michelle.cook@lakemills.k12.wi.us) to schedule both a mid-year assessment and an end-of-school year assessment. We recommend the student participate in Summer School to ensure the student is ready for the next grade level.

A preliminary screening will be completed by the student's family using the Ages & Stages Questionnaires: A Parent-completed, Child Monitoring System (ASQ).  Families will receive a link to complete the ASQ questionnaire online. If further information is required, teachers will reach out to families directly.

To enroll your child into the 4K or 5K program, simply click HERE to begin our online Enrollment Application. If you do not have access to a computer, please contact Mrs. Beckie Crenshaw to schedule an appointment in our office.  (We do not recommend trying to enroll from a mobile device!)

Once you have completed the Enrollment Application, your application will be processed.

Bus transportation for our 4K students is free.
